The following PDF forms are intended for filing in the Karuk Tribal Court. The forms have been made fillable so that they maybe downloaded, completed, printed and submitted, VIA FAX to the Karuk Tribal Court. You will receive written confirmation of the date and time of recorded receipt by the Clerk of the Court.
Any filing fees need to be delivered to the court within five (5) court days of the received written confirmation either in person or via US Mail.
Cash, Money Order, or Check are accepted and must be made payable to The Karuk Tribe. Credit cards are not accepted.
Please ensure that when filing VIA FAX, you have completed all forms thoroughly and include the Fax Filing cover sheet.
